I think so – he appeared on the second round. We started off just fighting some bugbears, then the giant arrived, followed by carnivorous apes. Whole fight took two hours, which was slow, even by our standards!

A thing that occurs to me-- I have never used the escalation die-- is that special moves like Dragon Age's stunts could be tied to it, too? I use the World of Darkness system for my fantasy game, & I'm just trying to think about it...like, maybe it would be a way to bring in exceptional successes or dramatic failures? Huh.

In 13th Age (which I'm in the process of reading but not played yet), all sorts of PC powers work with the escalation dice - for example, when the escalation dice is at 3+ you do an extra dice of damage with this attack. Pretty cool. In 4e I've found it speeds up combat and makes it less grindy (although not last night!)
